Either fresh or fake will work fine. If fresh,cover styrofoam with foil to limit mold so you can reuse it. You can paint either with spray paint or acrylic paint. Have fun and enjoy.

While I appreciate where she is coming from with this craft, I wonder if a real pumpkin could be used? It's a temporary decoartion anyway and pumpkins usually keep for quite some time, even after being cut. I think you could even paint a real one, couldn't you? Of course, if pumpkins are expensive where you are, it might not be worth it, but here we can get them really cheap when they're in season.
